Notice Under the Americans with Disabilities Act and the ADA Amendment Act

In accordance with the requirements of the Americans with Disabilities Act of 1990 (鈥淎DA鈥), the Maricopa County Community Colleges District (MCCCD) will not discriminate against qualified individuals with disabilities on the basis of disability in its services, programs, or activities. Furthermore, applicable law also prohibits retaliation against any employee who participates in a protected activity.

Who is Protected?

Employment discrimination is prohibited against 鈥渜ualified individuals with disabilities.鈥 This includes applicants for employment and all current employees. MCCCD does not discriminate on the basis of disability in its hiring or employment practices as put forth by the EEOC under Title I of the ADA. Who is Responsible for Compliance?

Human Resources is responsible for overseeing Maricopa鈥檚 compliance with the federal and state laws addressing disability discrimination and accommodation, as well as MCCCD policy and philosophy.

What is Reasonable Accommodation?

Reasonable accommodation is a modification or adjustment to a job or work environment that will enable a qualified applicant or employee with a disability to participate in the application process or to perform essential job functions. A reasonable accommodation is identified in the Interactive Process.

How to Request an Accommodation

Step one, employee submits request for employee accomodation and licensed healthcare provider questionnaire. Step two, 港彩开奖 HR and HR solutions center representatives evaluate the request based on the documents.Step three, employee and/or LIC health care provider may be consulted to gather more INFO.Step four, depending on the request (equipment,work/class schedule,etc), HR communicates with the party who can provide the accommodation. How to request

Employee ADA Accommodation Request Process

  1. Employee Submits Request for Employee Accommodation Form and Licensed Health Care Provider Questionnaire (LHCPQ)
    1. Employee submits Employee Request Form to 港彩开奖 HR. Licensed Health Care Provider sends LHCPQ to District HR Solutions
  2. 港彩开奖 Human Resources (HR) and HR Solutions Center (HRSC) conducts intake
    1. 港彩开奖 HR and HR Solutions representative evaluates the request based on the documents received
  3. 港彩开奖 HR and HRSC engages in interactive process with employee and supervisor
    1. Employee and/or Licensed Health Care Provider may be consulted to gather more information
  4. 港彩开奖 HR submits request to the appropriate party
    1. Depending on the request (i.e. equipment, work/class schedule, etc) HR communicates with the party who can provide the accommodation
  5. 港彩开奖 HR notifies employee
    1. 港彩开奖 HR notifies the employee (and supervisor if appropriate)

ADA

The ADA interactive process is intended to be a flexible approach that centers on the communication between MCCCD and the employee requesting reasonable accommodation. It involves obtaining relevant information from the employee and the employee鈥檚 health care provider.

  • Licensed Health Care Provider Questionnaire.

Confidentiality:

All medical information shared with the Maricopa County College District (MCCCD) through the ADA/ADAAA and/or reasonable accommodation process will be maintained separate from personnel files and confidential in accordance with State and Federal requirements.

Grievance:

MCCCD do not apply to claims that involve accusations of discrimination, harassment, or violation of federal or state anti-discrimination laws. Americans with Disabilities Act (ADA) is a federal law that protects people with disabilities from discrimination.
